The deal for Horford became possible when the 76ers agreed to a sign-and-trade with Miami for Jimmy Butler.
The 33-year old is now guaranteed a contract through his age 36 season.
Horford was the No. 3 overall pick in the 2007 NBA Draft that saw three Gators selected in the top- ten picks.
After leading Florida to their only two National Championships, the big man spent nine years in Atlanta before playing three seasons in Boston.
Horford has averaged 14.1 points and 8.4 rebounds per game over his career while shooting at a 52.5% clip.
